Southlake Health’s Patient, Family and Caregiver Declaration of Values and Service Charter reflect what matters most to our patients, families, caregivers, and teams — and reinforce our shared commitment to delivering leading edge care, close to home.

Together, they bring forward the voices and experiences of patients, families, caregivers, staff, physicians, volunteers, and community members. Our communities and teams helped shape the principles that guide how care is delivered and experienced at Southlake through extensive consultation and validation. Southlake is thankful to everyone whose feedback supported this important work.

What is a Declaration of Values? 

The Patient, Family, and Caregiver Declaration of Values is a shared statement that reflects what matters most to those receiving and providing care. It defines the values that guide how care should be experienced, shaping how we communicate, partner, and deliver care across every interaction. 

What is the Service Charter? 

The Service Charter complements the Declaration by outlining clear expectations for how care is delivered and how we work together. It helps ensure that interactions are respectful, transparent, and grounded in a shared understanding between patients, families, and care teams. 

How was this developed? 

This work was shaped through a structured and inclusive engagement process over several months, ensuring a wide range of voices helped inform the refreshed Declaration of Values and Service Charter. The refreshed Patient, Family, and Caregiver Declaration of Values reflect what matters most to patients and caregivers.
