Southlake launches new Population Health Management Advance

Thursday October 15, 2026

New initiative will accelerate Southlake’s strategy by developing new approaches to improve health outcomes, experiences, equity and health system sustainability

Southlake Health is launching a new initiative focused on finding ways to better meet the health needs of people and communities across northern York Region and southern Simcoe County. The Population Health Management Advance – or PHM Advance – will support clinical teams at Southlake in responding to changing needs across the communities it serves. Using data, insights, and partnerships, the PHM Advance will identify opportunities, develop and test new approaches, measure their impact, and scale what works.

Population Health Management is an approach to understanding the health needs of communities and using those insights to improve how care is planned and delivered. The PHM Advance will build on the leading edge care at Southlake, enabling teams to intervene earlier, before people require more complex services. By identifying patients who may benefit from additional support and connecting them with the resources and navigation they need, Southlake can reduce the need for hospitalization and contribute to a more sustainable health system.

“Meeting the needs of our growing communities requires us to think differently about how care is delivered,” said Paul Woods, President and CEO, Southlake Health. “Southlake has incredibly talented staff and physicians that provide outstanding, compassionate care every day. The PHM Advance will help us find new ways to improve the health of the populations we serve. This is about ensuring we can continue to deliver leading edge care today, while building a stronger, more sustainable health system for generations to come.”

The PHM Advance will examine emerging ideas and evidence in population health, with a focus on practical solutions. Built around a core team at Southlake Health, the PHM Advance will also work with local providers to help improve health outcomes at the population level, while making it easier for healthcare teams to connect patients with the care they need.

Southlake has brought together a Global Advisory Board of leaders from across Canada, the United States, the Netherlands, and Singapore to inform the work of the PHM Advance.

“Southlake has a well-established track record of leading health system transformation,” says Tyler Chalk, Executive Vice President and Chief Strategy Officer, Southlake Health. “The PHM Advance is an exciting next step for Southlake, bringing together the expertise of our staff and physicians with new ideas and perspectives. We are thrilled to have an exceptional group of leaders from around the world joining us on this journey and are grateful for their support.”

The PHM Advance supports Southlake’s ongoing transformation into a Distributed Health Network, a more sustainable model that brings care closer to home across multiple sites and communities rather than relying on a single hospital. The PHM Advance will serve as a platform to accelerate that work, as Southlake builds a more proactive, connected, and sustainable health system where people are supported to stay healthier for longer and access the care they need, when and where they need it.
