Welcome to Southlake Health! Please check in at the Welcome Centre to the right of the East Entrance upon arrival for your appointment. If you require mobility assistance please bring a support person with you. If your appointment is in the Medical Arts Building, check in at the Welcome Centre on Level 4 of the MAB.
Southlake is a mask friendly environment. Masks are available at all public entrances, including the Emergency Department, Cancer Centre and East and West entrances.

Please bring your Ontario Health Card and arrive 15 minutes prior to your appointment to allow time for registration. Upon arrival, you are required to register for your appointment at the Welcome Centre, and then you will be given directions on where to proceed to next.
