Employee and Family Assistance Program (EFAP)
Southlake staff can access our Employee and Family Assistance Program by phone or online.
Staff are typically offered an appointment within 2 – 3 business days and can usually see a counsellor in 3 – 5 business days.
Southlake’s Employee and Family Assistance Program is provided by Homewood Health Inc.

EZ Labour Portal
Logging in for the first time? Note: You will need your employee number, which you can find on your pay stub in the e-Pay portal.
Visit the EZ Labour Portal
In the Client Name field, input “SRHC”
Input your employee number in both the user ID field and the password field.
You will then be asked to create a series of security questions. You will need to answer these questions in the future should you ever need to reset your password.
Select a security question from the drop down menu.
Type the answer to this question into the field below the menu.
Repeat for the remaining 3 questions.
In the Old password field input your employee number.
Input your new password in the password field.
Then confirm this new password by retyping it in the confirm password field.

Southlake’s Corporate Brand Guidelines
To apply for a Southlake Staff Wellness Centre membership, please download the following two forms and label and scan them as separate documents to firstname.lastname@example.org.
- Wellness Centre Application package (see bottom of package for instructions on locating your prox #)
- Par-Q+ Health Questionnaire (to respect your privacy, please label and scan the Par-Q+ in a separate email from the application package)
For staff on Southlake’s payroll, membership costs $9 biweekly which will be deducted from your pay. Staff who are not on Southlake’s payroll are able to pay by credit card for a 3, 6 or 12 month membership as follows:
You will have the option to pay by Visa, Mastercard, or American Express by clicking on the online payment webpage.
- Type in Gym in the “Account” field
- Under “Invoice Date,” enter today’s date
- Under “Patient Name,” type in your name
- Under “Contact Phone Number,” enter your phone number
Under “Amount,” type in $58 for a three-month membership, $118 for a six-month membership or $238 for a 12-month membership. Two dollars will be added as a service fee to round up to the full amount.
Payroll deductions will continue until a cancellation form is submitted. If you need to cancel your gym membership, please complete the Cancellation package and email it to email@example.com. For staff paying by credit card, membership will automatically expire once the membership period is over, unless another payment is received for a new membership period.
The fitness area is approximately 1250 sq. feet and has a capacity of 40. The studio is approximately 600 sq. feet and has a capacity of 12 participants per class.*Classes may change without notice.
The Wellness Center Includes:
- Accessible single stall change room
- Day Use lockers (bring your own lock)
- Water Cooler (bring your own bottle)
- Main fitness area; two televisions with basic cable
- Studio; television complete with DVD
- Fitness stretch barres
- Camera Surveillance in Studio and Main fitness area
- Storage cubbies for belongings in Studio
- Southlake telephones
Fitness Equipment Includes:
- Three elliptical machines
- Three treadmills
- One stationary bike
- One adjustable pulley machine
- One dumbbell set (weight up to 50 lbs)
- Two benches
- Stability equipment (stability balls, yoga mats, Pilates balls, and resistance bands)
Membership fees include access to the fitness and wellness classes.

Application and Cancellation Info:
Enrolments will be processed as they are submitted. Staff will receive an email when their membership has been activated.
For staff paying by payroll deduction, cancellations will normally be processed on the first or second pay following submission of the cancellation form (depending on where we are in the payroll cycle when form is submitted). For staff paying by credit card, membership will automatically expire once the membership period is over, unless another payment is received for a new membership period.
Locating the Proximity # on the ID Badge:
Your proximity # on your badge is required in order to complete the registration. This 5-digit number can be found on the back of your ID badge.
Note: if there is a zero in front of the 5-digit number, please disregard it.
