Outpatients coming to the hospital for day procedures can plan for their visit with the following information.
What to bring
Please bring the following items to your first visit:

- Your Ontario Health Card (for your first and every visit)
- A list (or bag) of any medications (prescriptions/over-the-counter), vitamins, minerals and herbal supplements you are taking
- A list of your allergies
- Name, address and phone number of your family physician
- Name, address and phone number of your pharmacy
- Your health/drug insurance plan(s) card or its group and policy numbers [Not all recommended medications are covered by the Ontario Health Insurance Plan (OHIP). This information helps our staff do their best to help you receive reimbursement for medications OHIP doesn’t cover.]
- A list of any questions or concerns you may have
- Eyeglasses, hearing aids, and/or any other devices/equipment or supplies you use every day (e.g. ostomy bags, oxygen tank, insulin, if you are diabetic)
- Snacks, meals and water/drinks or money/debit/credit card to purchase them at the hospital’s cafeteria of one of its onsite vendors
- Any Advance Medical Directives, such as an Advance Care Plan or Power of Attorney for Personal Care, if needed
(Ask a family member or friend to help you gather these items, if needed.)
What to leave
- Do not bring or wear any perfume, cologne or scented products
- Do not bring any tobacco/e-cigarette products, as smoking/vaping is prohibited inside and outside the Stronach Regional Cancer and Southlake Health.
What to expect
Stronach Regional Cancer Centre
When you arrive at the SRCC at Southlake welcome centre on the lobby level, our receptionist will greet you, record your name, arrange for your photo to be taken and added to your personal health record for your safety. They will then direct you to the Outpatient Clinic on level one (1) for your appointment.
During this appointment, members of your care team will:
- Check your height and weight, ask you about your medical history and may have you complete some forms
- May give you a physical examination
- Review any current medications you are taking with you
- Discuss any additional tests you require and book them for you
- Provide you with information, answer your questions, discuss your treatment plan and upcoming appointments
