Requesting your health information
The Health Information Office at Southlake oversees the collection, storage and retention of patient health records. It respects and maintains the confidentiality of every patient’s personal health information and its custody according to government legislation and the corporation’s by-laws.
Ordering copies of your health records
When requesting your own personal health information Health Information provides patients with two options for ordering copies of your hospital records:
- Online using MedChart
- Online using PocketHealth
- Order by phone or email and then pick up your record
Order online using MedChart
To order your record online visit MedChart.ca.
Once your order is placed, Southlake’s Health Information staff will upload your records to your secure patient portal in MedChart, and then an email notification will be sent to you to let you know that your records are available.
You can then log into MedChart to securely share your record with other physicians, insurance companies, lawyers and other third parties with which you need to share your hospital records.

Press Option 1 to request your Diagnostic Images copied to a disk
Press Option 2 to request Release of Information – for legal, insurance, personal records, and WSIB
Press Option 3 for all other requests, including Circle of Care

The Health Information office is open Monday to Friday between 8 a.m. to 4 p.m.
Note: Health Record Services require the original signed authorization form, or the online authorization completed via MedChart.ca, in order to complete your request. Southlake fee schedule applies.
