Southlake Community Ontario Health Team launches a new website to support residents of northern York Region and south Simcoe County

Today the Southlake Community Ontario Health Team (OHT) launched the new southlakecommunityoht.ca, which has been redesigned to provide a user-friendly centralized website to share information and updates about our connected care initiatives with residents of northern York Region and south Simcoe County.

The new website will allow residents to easily access information about OHT programs, provide opportunities for engagement and highlight the various care options available to the residents we serve. Over the last year the Southlake Community OHT has implemented several new initiatives and programs to support our community members. The partners have worked closely together over the years to provide high-quality services to the patients, families and communities who rely on them for care and this revamped website will be an extension and reflection of this work.

The website includes information about Southlake Community OHT programs, resources for healthcare providers and how to join the OHT, as well as opportunities for both residents and health care providers to get involved with the OHT.

“This new website is a great example of the shared commitment of the Southlake Community OHT partners to create and continuously improve information, access and experiences for the people and communities we serve,” said Christina Bisanz, Chief Executive Officer, CHATS – Community & Home Assistance to Seniors and co-chair of the Southlake Community OHT Joint Executive Steering Committee. “The user-friendly format will allow community members to readily navigate information about the incredible OHT programs available to them, along with exciting opportunities for engagement and collaboration.”

“A new website will provide a unique space for our partners to voice their passion for our team and allow us to demonstrate to our community how we are all working together to bring them the best possible care for patients, families, and providers,” said Dr. David Makary, Southlake Community OHT Primary Care Lead and co-chair of the Joint Executive Steering Committee. “We invite our communities to visit the website, learn more about the Southlake Community OHT and how the programs it provides can support them.”
