Self Check-in is a service available for patients with scheduled appointments at select outpatient clinics (see list below) at Southlake Health. Self Check-in is available through the Convey mobile app. Use this app to:
- Register and check-in for your upcoming appointment(s)
- Verify your registration information is up to date
- Receive timely notifications and directions to the clinic(s)
To use this service, you must have provided your cell phone number and provided consent to send text messages for upcoming scheduled appointments. The Welcome Centre at Southlake is available if you prefer to complete registration in-person at the hospital.
The facility code “SRHC” will be required for access.

- Diagnostic Imaging: Bone Mineral Density Exam (BMD)
- Diagnostic Imaging: Ultrasound
- Diagnostic Imaging: Nuclear Medicine
- Diagnostic Imaging: Mammography
- Diagnostic Imaging: Magnetic Resonance Imaging (MRI)
- Diagnostic Imaging: Computed Tomography (CT) Scan
- Diagnostic Imaging: X-Ray (where walk-in appointment is not available)
- Respiratory Therapy – Level 5, Medical Arts Building
- Urgent Cardiology Clinic
The clinic appointment reminder (once you have provided your cell phone and consent to send messages) will be specific to the location of your appointment(s).
We are currently working with other programs and services to allow them to also use the appointment self check-in solution.
