e-Pay – Pay stubs

Our Employee and family Assistance Program (EFAP) is available to staff and their immediate family members.

Homewood Health is Southlake’s Employee and Family Assistance Program provider. They offer supportive and confidential care and will provide you with assessment and guidance for any challenge.

Southlake Staff can access the EFAP via phone 24/7, 7 days/week to book appointments for counselling or coaching or access resources and chat features online.

Extranet – Access the Intranet and SafePoint from outside the hospital

EZ Labour portal

Logging in for the first time? Note: You will need your employee number, which you can find on your pay stub in the e-Pay portal.

To create your password, follow the instructions in the video below. in either the video tutorial below or the instruction below the below the video.
Creating a password for the EZ Labor Portal: Step by Step
  1. Visit the EZ Labour Portal
  2. In the Client Name field, input “SRHC”
  3. Click Submit
  4. Input your employee number in both the user ID field and the password field.
  5. Click login
  6. You will then be asked to create a series of security questions. You will need to answer these questions in the future should you ever need to reset your password.
  7. Select a security question from the drop down menu.
  8. Type the answer to this question into the field below the menu.
  9. Repeat for the remaining 3 questions.
  10. Click next
  11. In the Old password field input your employee number.
  12. Input your new password in the password field.
  13. Then confirm this new password by retyping it in the confirm password field.
  14. Click submit.
You have now created your password.

Multi-factor authentication is required to sign-into hospital systems remotely.

Staff and physicians are required to self-screen for COVID-19 and notify Occupational Health of any symptoms. Please call the hotline at extension 2383. You can also call the hotline for advice.

Smart Commute – Find a carpool to work

Southlake Shout Out is a peer-to-peer recognition program designed to honour your hard work and dedication. The program provides an easy way to recognize your peers by sending two types of awards: 

  • Southlake Values Awards – recognize your coworkers for practicing Southlake’s Values (Always with compassion, Power of many, Serve with purpose, Every voice matters, Courage to think differently). 
  • Thank You Awards – send a thank you note to a colleague at any time for any reason. Saying thank you and recognizing the hard work of our peers is something we cannot do enough. 

Within your Southlake Shout Out account you will have the ability to manage your own peer group, allowing you to like and comment on your peers’ recognition activity. So far more than 250 recognition cards have been sent!

Our Staff Wellness Centre is open 24/7. You will just need to use your badge card to enter the doors of the MAB from the bridge after 9:30pm.

The Wellness Centre is located on the 1st floor of the MAB in the entrance facing the lobby. It has a full wall of south-facing windows that allows for natural light.

The Fitness Gym is a 1250 sq. feet area fully equipped with a variety of cardio machines and fitness equipment. The Studio space is 600sq ft and is equipped with chairs, yoga mats and other equipment for classes.

Under the current COVID-19 requirements – the max. capacity is 7 for the gym and 6 for the studio (as of Sept. 2022)

How to apply for a membership:

To apply for a Southlake Staff Wellness Centre membership, please download the following two forms, fill out completely and send to staffwellnesscentre@southlake.ca. Please send only one email with both forms attached. In the Subject line please put “Membership Application – YOUR NAME”

  1. Wellness Centre Application package (Please fill out completely, or your membership will be sent back to you).
  2. Par-Q + Health Questionnaire

Please Note: Your Proximity # on your ID Badge is a 5 digit number that can be found on the back of your badge. If there is a zero in front of the number, please disregard it.

Memberships are FREE for all staff, physicians, volunteers and students

When will you have access to the gym?

Enrolments will be processed as they are submitted. Staff will receive an email when their membership has been activated.

Cancellation of Membership (Please read):

Please note: You must email staffwellnesscentre@southlake.ca with the following information:

  1. Full Name and employee ID number
  2. Your request for cancellation of the membership
  3. The date you wish the cancellation to start.

THE WELLNESS RESOURCES:

  • Fitness Gym –  2 TV’s with basic cable
  • Studio – TV and DVD
  • Accessible single stall change room
  • Day Use lockers (bring your own lock)
  • Water Cooler (bring your own bottle)
  • Camera Surveillance for safety in both areas
  • Storage cubbies for belongings in Studio

  • Cable pulley exercise machine

  • Mindful Breath Meditation – Mondays 07:50 – 08:00
  • Guided Meditation – Wednesdays 12:15 – 12:40
