The Tannery Mall, 465 Davis Drive
The Psychogeriatric Assertive Community Treatment Team (PACTT) is a Southlake Regional Health Centre community team located in the Tannery Building on Davis Drive. Under the guidelines of ACT Ontario, the PACTT program services clients who are struggling with a mental illness and who need intensive long term treatment and support.
The goals of ACT teams are to minimize hospitalization and to improve quality of life to clients in their own homes and communities. PACTT is an interdisciplinary team, comprised of Registered Nurses, Social Workers, an Occupational Therapist, a Recreational Therapist, Psychiatrist and a Program Assistant. PACTT provides a wide array of supports and services to clients and their families, relating to their mental health challenges.
Admission Criteria
- Clients must be 55 years of age or older.
- Clients must have severe and persistent mental health illness with psychosis, such as Schizophrenia, Bipolar Disorder or major depression.
- Recent history of multiple hospital admissions and psychiatric treatment.
- Clients and or the substitute decision-makers must be agreeable to PACTT services.

Admission Criteria
- Clients must be between 18 – 65 years of age.
- Clients must have a primary diagnosis of mood disorder (depression or bipolar disorder), anxiety, schizophrenia or schizoaffective disorder.
- The client has a need for medication adjustment and short-term follow up but does not require hospitalization. If the client is experiencing very acute distress, active suicidal ideation, or extreme aggression, they should be directed to emergency care
- The client does not require long-term follow up.
- The client is not currently followed by a psychiatrist and must be referred by a family physician with admitting privileges at Southlake.
The RAPT Clinic does not provide psychotherapy. We may be able to recommend free and fee-based services in the community.

This outpatient program is designed to provide assessment and treatment planning for individuals living with schizophrenia or schizoaffective disorder.
The Schizophrenia Program includes individual, group and family education, as well as medicine administration and monitoring.
Admission Criteria
- Clients 18 years or older with a diagnosis of schizophrenia or schizoaffective disorder
- Clients referred with other psychotic disorders will be considered on a case-by-case basis

Clients referred must have a psychiatrist with admitting privileges at Southlake and/or have affiliation with the clinic.

The Assertive Community Treatment Team (ACTT) is centered in Keswick, Ontario. It is offered 24 hours a day/7 days a week and aims to provide intensive services for people with severe mental illness in the Georgina and Newmarket areas. The goals of this team include:
- Lessen or eliminate the debilitating symptoms of mental illness, and to minimize or prevent recurring episodes of illness.
- Meet the client’s basic needs and enhance their quality of life.
- Improve behaviour in adult social activities and employment roles.
- Lessen the family’s responsibility of providing care.
- Decrease the number of hospital visits, emergency visits, and crisis service contacts.
The ACTT can deliver these treatments and services in the client’s choice of environment.

- Clients must be 18 – 54 years of age
- Clients must have severe and persistent mental illness with psychosis such as schizophrenia, schizoaffective or bipolar disorder with a past history of multiple hospital admissions and psychiatric treatment.

Southlake’s Benign Hematology Clinic concentrates on benign hematology investigations for various blood disorders such as anemia, thalassemia and ITP.
The Thrombosis team investigates and manages patients with suspected or documented deep thrombosis (DVT), pulmonary embolism (PE), or transient ischemic attack (TIA). Consultation for thrombophilia concerns, patient education, monitoring and management of anticoagulant dosing by a certified anticoagulation pharmacist, are all part of the services provided at this clinic.

When your baby’s bilirubin levels are high, a condition called jaundice can occur. Testing the level of bilirubin is done by a blood test. Serum bilirubin levels are drawn and the results are sent to the ordering physician. A nutritional assessment is completed and reviewed with parents, and follow-up instructions are discussed. This test can only be performed by a physician who has privileges at Southlake.

Southlake’s Birthing Unit offers private birthing rooms where mothers labour, birth, and recover in a warm and comfortable setting. Each room is spacious and includes a three-piece washroom and a pull-out chair for family members to spend the night. Modern operating rooms and a recovery room are located within the Birthing Unit for both elective and emergency caesarean sections.
Birthing Room Guidelines
All labouring women are admitted to a private birthing room to labour and birth. Following birth, we make every effort to give new mothers an available room that matches their insurance coverage. All mothers will be given the option to pay to upgrade from a ward or semi-private to private room accommodations. Private rooms are available in the Birthing and Post-Partum Units.
Note: your baby’s safety is an important part of our care at Southlake. Hospital staff, physicians, and volunteers can be easily identified by their photo identification badges. It is mandatory for all of these individuals to wear their ID badge while at the hospital. If you require assistance, observe a suspicious person or activity, have lost personal items, or have any security concerns, inform a staff member immediately.

Chemotherapy Program Clinic
Chemotherapy (chemo) is a treatment that uses drugs to damage and stop cancer cells from growing. You can take chemo drugs as a pill (orally), by injection with a needle or into your vein (intravenously). The cancer’s location, treatment goal, type of drug and other factors help your care team decide the best way to give your chemo.
Chemo is usually given in cycles or repeated doses. This means you will receive medication followed by a period of rest, which gives your body time to replace damaged cells and regain strength.

The Eating Disorders Program has been identified as a regional leader in assessment, treatment, and professional training for adolescent eating disorders in the Central East region of Ontario including Peterborough, Haliburton, Durham Region, Simcoe County, Kawartha, Pine Ridge, and York Region.
The Program is affiliated with the University of Toronto and York University for ongoing research.
Outpatient Program
For those clients who are able to manage and function sufficiently well in the community, services offered may include one or more of the following:
- Individual therapy
- Family therapy
- Nutritional counseling
- Psycho-education classes for families
- Parent support group, and/or
- Therapeutic groups for adolescents.
Inpatient Program
The Eating Disorders Program provides inpatient services for up to two patients in a specialized Child and Adolescent Mental Health in-patient unit. Access to beds is made as transfer requests from other programs and facilities, the Emergency Department, and from Child and Adolescent Mental Health Outpatient Services of Southlake.

The Child and Family Clinic is a regional outpatient service for children up to 18 years of age who live in York Region. Referrals from physicians and secondary service providers (with the knowledge and agreement of their family physician) are accepted for children and adolescents with severe mental health difficulties and their families.
The Clinic
The Child and Family Clinic is an outpatient, child and adolescent mental health clinic, that serves northern York Region and southern Simcoe County.
Our mandate is to provide assessment and treatment services for the most severe mental health problems of childhood and adolescence.
Staff is comprised of a team of professionals from the disciplines of psychiatry, psychology and social work, including Master’s level social work students.
The clinic is open Monday to Friday.

What to expect
The Child and Family Clinic provides psychiatric consultations for all new clients, followed by specialized psychotherapy services where indicated. Capital duration of service is time limited and goal oriented. The Clinic can offer 10 to 12 sessions, determined by the clinical needs of each client and his/her family. Ongoing evaluation of the client’s progress is made during the service period between the clinician and the client/family.
We provide service for children and families between the ages of 6 to 18. He/she must have had a recent Paediatric assessment completed.

The clinic is not a residential or day treatment facility. Services are provided on an outpatient basis only.
All services are fully covered under OHIP.
Requests for Psychiatrist Consult must be made must be made by the family’s physician. Recommendations from the Psychiatric consult may be to our clinic or to the community. The request for Psychiatric consult willbe reviewed by our Intake and processed for anappointment or redirected. Referral form mustinclude parent(s) contact infomation and custodystatus. Cases before the court will be declineduntil resolution.
Cancellations require a minimum of 2 business days’ advance notice. lnadequate notice could result in a fee to the client or termination of service.
Once the referral has been made, receipt of the referral will be confirmed with the referring doctor with the estimated wait time, to be shared with their patient. Intake will contact the patient/family to book the consultation within 2-4 weeks of the appointment.

A community treatment order is a six-month treatment agreement which facilitates an intensive community-based care plan for individuals with a serious mental illness. The focus of the agreement is to provide support, case management, maintain wellness, and reduce hospitalization.
The treatment agreement is voluntary and treatment orders can be renewed after the six-month period.
Admission Criteria
The patient must have had at least two admissions in the last three years. Or an admission of 30 days or more in the past three years.

The Coronary Intensive Care Unit (CICU) is staffed by a specially trained interprofessional team that provides critical care to acutely ill patients experiencing cardiac conditions requiring advanced diagnostic testing, monitoring, and treatment. This includes percutaneous coronary intervention (angioplasty), cardiac surgery or heart rhythm procedures.

The Day Hospital Program serves youth from 14 to 18 years of age who live in York Region. The program provides services for clients with severe psychiatric disorders which are hindering their ability to function in certain areas of their life.
The Day Hospital Program receives internal referrals from other Southlake Child and Adolescent Mental Health Programs or external referrals made by the family’s physician.
The program is approximately 24 weeks in duration. The program offers an eight space specialized Care and Treatment classroom and intensive psychotherapy services provided by a multidisciplinary team and a Special Education Teacher from the York Region District School Board.

During regular hours, the Emergency Department has a discharge planner. Their job is to guide you and your family through problems related to hospital admission and discharge services. Discharge planners also assist you and your family to understand and manage the effects and complications of your illness. Southlake encourages you and your family to follow up with the discharge planner by phone if needed.

When a pregnancy ends unexpectedly — prior to 20 weeks — women are referred to the Early Pregnancy Loss Clinic (EPL) through the Emergency Department or their family doctor. Options and treatment will be discussed and planned, as well as necessary follow-up.

The Fast Track area of the Emergency Department operates from 10 am to midnight, seven days a week with dedicated clinician coverage. This area is another assessment space available in the Emergency Department for patients to be seen and treated appropriately. Often patients with less urgent conditions will be seen in this space.
Our Fracture Clinic’s orthotech applies and removes casts 11 a.m. to 11 p.m., seven days a week.

The Fracture Clinic team consists of orthopaedic surgeons, orthopaedic technologists, physiotherapists and occupational therapists who work together to assess and treat patients with post-operative conditions, fractures and soft tissue injuries.
A referral from either an Emergency Department physician or their orthopaedic surgeon is required to be seen in the Fracture Clinic.

Southlake’s Geriatric Assessment Clinic (GAT) investigates and manages the cognitive and behavioural irregularities for patients 65 years of age or older. GAT professionals include nurse specialists, occupational therapists, and clinical pharmacists that work together in order to help support and educate our patients, caregivers and families.
The GAT Clinic operates two days per week.

A Geriatric Emergency Management (GEM) nurse is available to the Emergency Department if there is an apparent need. A GEM nurse’s focus is on seniors who are at risk of suffering an injury, in need of assistance, or who are pending admission to long-term care. They also provide support to the Emergency Department in developing an elder-friendly emergency healthcare environment.

An upper gastrointestinal (GI) endoscopy is a procedure that involves using a long, flexible tube to see inside the upper digestive system. Most of the time this will entail the esophagus, stomach and small bowel. The GI endoscopy program is managed by Southlake’s surgical program and operates from the Medical Arts Building (581 Davis Drive) and our Ambulatory Day Care (East Building, level 1).
Hand Program
Hand Program
The primary goal of the Hand Program is to provide comprehensive assessment and treatment of hand and upper extremity injuries, such as hand therapy and work therapy. The program’s healthcare team consists of an orthopedic surgeon, plastic surgeons, occupational therapists, and consultation with physiotherapy as needed. Four of the therapists have achieved the designation of a Certified Hand Therapist.
Note: A doctor’s referral is required to see one of the Hand Program’s occupational therapists.

The program’s occupational therapists provide assessment and intensive treatment to patients with acute hand injuries. Problems most commonly treated in the Hand Program are hand and wrist fractures, tendon and nerve lacerations, ligament strains and tears, and repetitive strain injuries.

The RN Health Coaches:
- Listen to a patients’ stories and experiences
- Create a supportive and judgment-free environment
- Help focus on the patients priorities and goals
- Help patients to navigate the Health System and available resources
- Understand the challenges of living with chronic illness and the impact of those challenges on a patient’s self management.
RN Health Coaches can help patients to explore the “first steps” to meeting these goals. They understand that people live their lives differently and experience different challenges while trying to balance life and management of their health challenges.

Referrals are easy to make and can be made by any member of a Healthcare Team, the patient themselves, or family members.
Referrals can be made for individuals who:
- are living with a chronic illness (diabetes, heart disease, lung disease, arthritis)
- are experiencing frustration with balancing daily living and managing illness
- have been labeled “non-compliant”

Southlake’s Heart Rhythm Program cares for people with irregular heartbeats, or arrhythmias; conditions such as atrial fibrillation, tachycardia, flutter, a slow or racing heart, dizziness, or Wolf Parkinson White syndrome.
The Heart Rhythm Clinic provides ongoing assessment to individuals with permanent pacemakers and Implantable Cardioverter Defibrillators (ICD), as well as programs and monitors pacemaker batteries to ensure their safe functioning. Follow-up visits give registered nurses, who are trained in cardiac pacing, the opportunity to provide valuable education to patients and their family.

Palliative care refers to the care of patients and their families who are facing a serious, life-limiting illness. Palliative care is patient-centred coordinated care that aims to relieve suffering and improve quality of life for patients and their families at all stages of the illness.
The Hospice Palliative Care Team (HPCT) is the Regional Palliative Care Program for south Simcoe County, York Region and North York. This compassionate, highly-skilled team works together to bring care directly to patients wherever they may live, including their home, retirement home, group home or long-term care residence.
Any patient who is receiving a palliative approach to care and requires assistance with symptom management can be referred to the HPCT. To refer a patient, please fill out the Palliative Common Referral Form on the Ontario Health AtHome website and fax the completed form to the HCPT.

Interventional cardiology deals specifically with catheter-based treatment of structural heart diseases. The main advantages of this approach are the avoidance of the scars and pain, and long post-operative recovery. Primary angioplasty, an interventional cardiology procedure, is now considered the gold standard of care for an acute myocardial infarction.

This is where patients who have hip, knee and shoulder surgeries are admitted.
Care often will differ from patient to patient. Some patients will experience common discomforts. Others will have complex needs. Your inpatient surgical team will work with you and your family on developing the best treatment plan.

At Southlake, we understand that compassion and exceptional care is essential for babies who require a higher level of care. Our Neonatal Intensive Care Unit (NICU) has medical expertise, progressive technology, and state-of-the-art equipment to provide this level of support.
As a Regional Level 2C nursery, our highly trained nursing staff and physicians are able to stabilize and care for babies born at 32 weeks or greater from conception to delivery — often referred to as preterm babies. Other full-term babies may require close monitoring for breathing and nutrition after birth at the NICU.
The NICU is comprised of an interprofessional team of intensive care providers with a dedicated team of volunteers to support new families during this difficult time.
To offer continued support to parents with babies in our NICU, courtesy accommodation is available for mothers whose babies must remain in the NICU after mom is ready for discharge.

Infants in the NICU will be given the opportunity to be held skin to skin by a parent or caregiver to promote infant physiological stability and positive parental psychological benefits. The practice of skin-to-skin contact between the infant and the parent/caregiver will be considered routine practice in the NICU.
Infant benefits include the following:

- Longer periods of quiet sleep
- Improved self-regulation development
- Increased breastfeeding incidence and duration
- Decreased pain perception
- Reduced stress and crying
Parental benefits include:
- Enhanced parent-infant attachment with a reduction in the negative effects of having a sick or preterm infant

Occupational therapy helps to improve and solve the problems that interfere with a patient’s ability to live a healthy lifestyle. They include self-care, being productive and the ability to perform leisure activities. Occupational therapy can also prevent a problem or minimize its effects.

The Southlake Paediatric Complex Care Clinic offers holistic wrap-around care for children with medical complexity, and their families through our partnership with SickKids. The goal is to bring resources from both hospitals directly to the patient, providing coordinated one-stop care for families while developing a long-term and supportive relationship with them throughout their health care journey.
Children who have complex medical conditions, such as cerebral palsy, congenital heart disease and children who do not have a clear diagnosis are considered to have complex care needs. As these children tend to be treated by more than one health care provider the Complex Care Clinic provides a coordinated, comprehensive, and collaborative model that is designed to fit each patient’s unique needs. All referrals to the Complex Care Program are triaged through SickKids.

Palliative care is a team approach to care that is appropriate for a patient and/or family member living with a life-limiting illness or needing end-of-life care. Palliative care relieves suffering and improves quality of life for people of any age and at any stage in a serious illness. Palliative practices are present whether the illness is curable, chronic or life threatening. Our palliative care practices provide each patient and their loved ones with respect, dignity, and an individualized care plan that meets their specific needs.

We deliver cancer care for children, from birth to 18-years-of-age, through our partnership with the Pediatric Oncology Group of Ontario (POGO) and SickKids Hospital. The Post-Partum Unit is a 20-bed unit that focuses on assisting new families following the birth of their baby. Staff on the unit help new families prepare for their return home and educate them on how to care for both mother and baby. This unit also supports undelivered mothers requiring hospitalization due to complications of their pregnancy until they are ready for discharge.

The Prenatal Assessment Clinic provides a full range of consultative and diagnostic services for patients who do not have a family physician. Patients are followed by the Obstetrician on-call and a Registered Nurse until the transfer of care to an assigned obstetrician at 16 -18 weeks of pregnancy. The clinic also offers all prenatal screening and referral to any other identified service.

Psychosocial oncology is a whole-person approach to cancer care. Our Psychosocial Oncology Program team has specialized training and expertise in oncology. Depending on your needs, your team may include a combination of: registered dietitians, a medical psychotherapist, nurses, nurse practitioners, pharmacists, psychiatrists, and social workers.
We offer you a range of clinics and support group programs to improve your quality of life throughout your treatment and beyond. Clinics are held on Level 1, and most support group programs are held in our large meeting room on the Lobby level of the Cancer Centre.

Stronach Regional Cancer Centre
Radiation therapy uses high energy x-rays to damage and stop cancer cells from growing. Getting radiation is like having an x-ray — you will not see or feel anything, and you won’t become radioactive. Watch the following short video to learn about radiation therapy at Cancer Centre and what to expect.
Your radiation therapy involves the following steps, which all take place on the ground floor (G) of the Cancer Centre:
1 – Pre-Radiation Patient Appointment – You will attend a 20-minute, one-on-one education appointment with a radiation therapist to tell you what to expect at every step.
2 – Planning Appointment – You will attend a one-hour planning appointment, where you will be given a computed tomography (CT) simulation. This appointment will give your radiation oncologist and radiation therapist the information they need to accurately outline your treatment area.
3 – Treatment Delivery Appointment – You will go to the Ground Floor for your radiation treatment. Your first treatment will be the longest (30 – 45 minutes). We will monitor you throughout your treatment and during weekly visits with your care team.

The investigation and management of Transient Ischemic Attack (TIA) and/or Cerebrovascular Accident (CVA) is performed at Southlake’s Stroke Prevention Clinic. Also, the clinic provides assistance in the primary and secondary prevention of stroke.

Taking Care of Pain
Taking Care of Pain
Following your surgery, pain can interfere with your ability to move and can result in a longer hospital stay. In order for your health team to understand how you feel, you will be asked to rate your pain on a scale from zero to 10. We will work with you to keep you as comfortable as possible during your hospital stay by providing you with pain medication, which can be given a number of different ways. Managing your pain by taking pain medication regularly is important, as it allows you to more effectively participate in your rehabilitation therapy.

The Eye Institute at Southlake and its Cataract Program offer patient-focused surgical eye care. We combine a patient-friendly environment with the latest technology and surgical techniques. The Eye Institute at Southlake is firmly committed to clinical excellence by experienced Royal College-certified ophthalmic surgeons, and a highly-trained team of anaesthesiologists, nurses and support staff.
The Eye Institute’s Cataract Program utilizes the latest diagnostic equipment and techniques to replace your clouded eye lens with a new corrective one. Patients are provided with a wide range of advanced intraocular lenses, some of which reduce the patient’s need to wear glasses following their surgery. The procedure is performed with minimum discomfort and patients return home the same day.

Thoracic Surgery School

In preparation for thoracic surgery (surgery to treat diseases of the chest and lungs), patients are referred to Thoracic Surgery School by their surgeon. A qualified team of nursing, physiotherapy and pharmacy representatives conducts this outpatient service. It includes an overview of the anatomy and function of the lungs, the location of incisions, breathing and coughing exercises, pain control management, and the importance of early mobility. Following surgery, patients reconnect with their Thoracic Surgery School physiotherapists in order to receive post-operative rehabilitation.
